Care Navigator/Diabetes Educator - Clinic
SUMMARY:

Care Navigators/Diabetes Educators are responsible for coordinating patient care and serving as a bridge between patients and healthcare services.

QUALIFICATIONS:

RN, along with 1-3 years of experience in clinical services settings.
Certified Diabetes Educator (CDE) or working towards becoming certified.
Communicate in an effective manner, follow written or verbal instructions and demonstrate good interpersonal skills to deal with team members, patients and families
Competent in EHR, Microsoft Office, Word, Excel, Outlook, and PowerPoint.
Excellent problem solving and critical thinking skills
Maintain discretion and confidentiality in communications.
Familiar with policies and procedures regarding reporting and release of protected health information in accordance with HIPAA regulations.
Proficiency in clinical documentation and reporting.
Possesses strong motivation and independence, with excellent organizational skills to ensure all job functions are completed efficiently and effectively.
Project a positive attitude toward job responsibilities, medical providers, coworkers, and administration.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Patient Education & Support Provider:
Deliver one-on-one and group education to patients with diabetes management, covering topics such as diet, exercise, medication, and self-monitoring of blood glucose levels.
Individual care planning: assess patients' needs, develop and adjust personalized care plans, and monitor progress over time.

Collabration:
Work with nurses, dietitians, and providers to coordinate comprehensive care.

Advocacy and support: Advocate for patients' rights, provide emotional support, and help navigate healthcare systems.

Community Outreach: Participate in prevention and education programs to raise awareness and promote healthy lifestyles.
Quality Improvement: Contribute to research, apply evidence-based practices, and participate in quality improvement initiatives.

Documentation: Maintain accurate up-to-date patient records and stay current with diabetes management guidelines.
EDUCATION/CERTIFICATIONS:

Registered Nurse

Certified Diabetes Educator (CDE)

LANGUAGE

Communicate verbally and in writing in English

WORK ENVIRONMENT: 

Regularly required to stand, walk and sit. 
Regularly required to reach with hands and arms. 
Regularly required to perform repetitive data entry. 
Regularly required to view computer monitors.  
Frequently be able to lift 25 pounds from the floor to waist level.
